KASIMIR is a decision support system for the diagnosis, treatment and follow up of cancer, particularly breast cancer.
KASIMIR is designed to be used by members of ONCOLOR, the regional cancer care network for Lorraine, where it is currently being evaluated.
The project is a collaboration between the following partners: LORIA (Laboratoire lorrain de Recherche
en Informatique et ses Applications); ONCOLOR (le réseau de soins en cancérologie de la région Lorraine);
Centre Alexis Vautrin, centre Régional de Lutte contre le Cancer de Lorraine;
CNAM (Le laboratoire d'ergonomie du Conservatoire National des Arts et Métiers de Paris).

This paper presents the KASIMIR research project for the management of decision protocols in oncology. A decision protocol is a kind of decision tree implemented in an object-based representation formalism. A reasoner based on such a formalism and on hierarchical classification is coupled with a knowledge editor. This association provides support for editing and maintaining protocols, enabling the detection of errors and the comparison between versions of the protocol. In this way, management of protocols takes fully advantage of the underlying knowledge representation and reasoning tools. This straightforward use of the protocol may be insufficient in some situations. Then, the protocol may have to be adapted for these situations. A study of protocol adaptation is presented. In particular a reasoner based on a combination of hierarchical classification and fuzzy logic is introduced.

Le réseau lorrain de cancérologie Oncolor propose des référentiels de bonnes pratiques pour le diagnostic, le traitement et la surveillance des cancers. En plus du support papier, deux supports informatiques, le site internet du réseau et un logiciel KASIMIR ont été développés pour faciliter la mise à disposition de ces référentiels à distance et leur actualisation. Cette étude a pour objectif d’évaluer ces trois supports dans le cancer du sein et de la prostate et de déterminer si les supports informatiques apportent une aide supplémentaire et une valeur ajoutée. De mai à juillet 2001, 25 médecins du réseau Oncolor ont analysé, en consultant les référentiels sur chacun des trois supports, 23 cas cliniques de cancer du sein et 22 cas de cancer de prostate, soit au total 1 293 évaluations réalisées. On observe pour le cancer du sein un taux de réponses conformes au référentiel significativement supérieur avec le logiciel KASIMIR comparé au papier (87,1 versus 77,8 %) ou à internet (87,1 % versus 75,8 %), une optimisation du recours à l’avis d’un confrère spécialisé ou d’un comité de concertation pluridisciplinaire et, globalement, une bonne satisfaction des utilisateurs. Ces résultats nous incitent à poursuivre l’expérimentation de KASIMIR, à le développer dans d’autres cancers, en particulier ceux dont la prise en charge est complexe, et à évaluer sur le terrain son impact sur les pratiques médicales."
Oncolor, the French regional cancer network of Lorraine, proposes clinical guidelines for the diagnosis, the treatment and the follow-up of cancers. In addition to paper documents, two computerized supports, a web site and the KASIMIR software have been developed to make the use of these guidelines and their updating easier. This study aims at evaluating these three tools in breast and prostate cancer and to determine if computerized supports provide an additional help and an added value. From May to July 2001, 25 physicians of the Oncolor network have analysed, by consulting the guidelines on each of the 3 supports, 23 clinical cases of breast cancer and 22 cases of prostate cancer, i.e. in total 1,293 cases. We observe for breast cancer a response rate consistent with the guideline significantly higher with the KASIMIR software compared to paper (87.1% versus 77.8%) or to Internet (87.1% versus 75.8%), an optimisation of the recourse to a specialised physician or a pluridisciplinary committee and globally a good satisfaction of the users. These results induce us to continue with the KASIMIR experiment, to develop it for other cancers, especially those with a complex management and to evaluate its impact on medical practices.
