The system "generates a Bayesian ranked differential diagnosis based on signs, symptoms, laboratory tests, country of origin, and incubation period and can be used for diagnostic support and simulation of all infectious diseases in all countries. In a blind trial done on 495 patients, the correct diagnosis was included in the differential diagnosis list in 94.7% of cases (sensitivity) and displayed as the first disease in the list in 75% (specificity).

"The data in GIDEON are derived from all peer-reviewed journals in the fields of infectious diseases, pediatrics, internal medicine, tropical medicine, travel medicine, antimicrobial pharmacology, and clinical microbiology; a monthly electronic literature search based on all relevant terms in GIDEON (e.g., diseases, drugs, etc.) all available health ministry reports (both printed and electronic); standard texts; and abstracts of major meetings."
"GIDEON is a computer program for diagnosis and reference in the field of tropical and infectious diseases, epidemiology, microbiology and antimicrobial chemotherapy."
"It was designed to diagnose all the worlds' infectious diseases, based on symptoms, signs, laboratory testing and dermatological profile. Gideon's infectious disease network pays special attention to the country of origin. The database incorporates 327 diseases, 205 countries, 806 bacterial taxa and 185 antibacterial agents.
GIDEON is made up of 4 basic modules:
- Diagnosis: GIDEON's diagnostic module enables the user to access all epidemiological parameters, clinical hints, diagnostic tests and optimal therapy. It involves data on conditions totally unfamiliar to most physicians outside the involved countries. After specifying the suspected country of disease, acquisition and entering the signs and symptoms of the patient, GIDEON provides a ranked list of differential diagnosis. For each disease on this list the user can retrieve important facts like its clinical and epidemiological profile, its status in the country, etc. By interacting further with GIDEON, the user can explore alternative diagnosis.
- Epidemiological Module: With GIDEONS Epidemiological Module the user can retrieve a diseases epidemiological parameters, access the status of any disease in the current country, get a list of the world wide distribution, review the status of the current disease in any country and the possibility to access a list of alternative names for a disease. The user can select a pathogen group, reservoir, vector, vehicle and/or country and list all of the diseases that fit these parameters, and he can review the status of global impact diseases like malaria, Aids, Yellow Fever, Cholera and Tuberculosis.
- Therapy Module: This module provides detailed information about choices in drug therapy, including susceptibility, toxicity and drug interaction data, not only on familiar antimicrobials, but also on ones rarely used. A list with synonyms is included of all commercially available antimicrobials worldwide. Vaccine and Immune Globulin information is similarly available.
- Microbiology Module: For purpose of identification and characterization, the Microbiology Module provides full laboratory characteristics including biochemical test results and cultural characteristics, for almost 900 organisms. With this Module, the user can identify and compare organisms through an encyclopedia of information all in one integrated source.

A Computer-Driven Bayesian Matrix For
The Diagnosis Of Infectious Diseases.
[gideononline]
|
"
Interactive computer data bases representing rates and clinical
probabilities were constructed for 289 infectious diseases; 115
symptoms, signs & laboratory findings; and 205 countries. User
input is processed by a Bayesian matrix, and compatible diagnoses
are presented in order of probability, with interactive
'suggestions' for additional discriminative examinations.
In a multicenter pilot study of 495 blinded cases, correct
diagnoses were included in the program's differential diagnosis
list in 469 (94.7%); and the correct diagnosis was ranked first in
350 (70.7%). The accuracy of diagnosis was highest for parasitic
disease (p .04) and diseases acquired in Africa (p .04) and South
East Asia (p .03); no such differences were noted with respect to
body system and patient age group.
